Lewis Animal Hospital was established in 1914 by Dr. Tim Lewisâ grandfather. The original practice location was in Carthage. Dr. Tim Lewis graduated from the Ohio State University College of Veterinary Medicine in 1974. He joined the practice and ran it until his retirement in 2010. Dr. Les Polasky graduated from the Ohio State University in 1988. He joined the practice in 1990 and eventually became a partner. Upon Dr. Lewisâs retirement he became the sole owner of the practice.
